The Prince George Council of Seniors is hiring for the following position:

Compassionate Dementia Inclusive Communities Program Coordinator  (CDIC- Community Partner) 

Reports to: Community Programs Team Lead (Hospice), Friends and Family Caregiver Support Program Coordinator (PGCOS), Executive Director (PGCOS).

Current Incumbent:  1-year term.

Wage: $25.18 an hour.  

Start date:  As soon as possible.

ROLE DESCRIPTION: The Compassionate Dementia- Inclusive Community Program Coordinator is a full-time position which lead the development, implementation, and evaluation of a dementia-friendly initiative that fosters awareness, inclusivity, and accessibility for individuals living with dementia and their care partners. This role is pivotal in building partnerships, engaging communities and delivering person-centred programs that reduce stigma and promote social inclusion.

KEY RESPONSIBILITIES:  

  • Program development & Launch: design and implement the Compassionate Dementia- Inclusive Community (CDIC) Program from the ground up.
  • Develop a strategic program framework with Collaboration with program partners.
  •  including goals, timelines, evaluation tools, and impact metrics.
  • Lead the creation of inclusive engagement materials and educational resources.
  • To recruit, train and retain volunteers for the CDIC program. 
  • Intake caregivers of those living with dementia.
  • Provide support to caregivers of those living with dementia.
  • Participate in, and co-plan, community events. 
  • Participate in community café at project end.

ADDITIONAL RESPONSIBILITIES:  

  • Member of PGHPCS Advisory Committee.
  • Attend biweekly meetings with UBC, and other CDIC sites.
  • Reporting of program activities monthly to PGHPCS.
  • Regular meetings with PGHPCS.
  • Participate in evaluation with UBC.

FUNCTIONAL REPORTING RELATIONSHIPS: 

THE CDIC Program Coordinator reports to their FFCSP Coordinator at Prince George 

Council of Seniors and works in partnership with PGHPCS. 

Qualification and Competencies

Required Education and Experience: 

  • Degree/Diploma in Social work, or related field (i.e. Healthcare, Psychology, Geriatrics), 
  • related experience in grief/palliative care. Experience in working with seniors and/or seniors living with dementia will be considered.
  • Valid driver’s license, reliable vehicle.
  • Demonstrated knowledge and ability to facilitate and coordinate programs.
  • Experience with volunteer recruitment and supervision.
  • Excellent communication skills. 
  • Ability to work effectively in a team based decision-making environment.
  • Self-directed, self aware, and professional.
  • Compassionate care provider and leader.
  • Proficiency in MS office, Google workspace and other virtual meeting platforms. 

PREFFERED ASSETS:

  • Familiarity with Dementia- friendly community’s model.
  • Lived experience as a caregiver or support worker.
  • Previous experience working in a nonprofit or social service setting.
